Ok, please don't get pissed at me for this, I have searched high and low for an answer and actally got 2 answers. But, I want a no-swap mod in my PS2... here is the info on it

Model SCPH30001 R
Serial U4093393
Date Code 2A
8 screws on the bottom

I have been told it is a version 5 and a version 4. Any help is appreciated!

Take the expansion bay cover off the back mate - there should be 2 gold coloured screws there - if there's also a silver one just to the right you have a v5 otherwise its a v4.

Ok, I notice that there are 2 gold screws, and a silver one... but the silver one is not "just to the right" But, by calling me mate, I am assuming you aren't from USA - and us yankees over here know that the rest of the world doesn't know their rights from their lefts!!! j/k

Most likely a Version 4. Is there kind of a square hole in the top? If its a V5

If there are 3 screws in the expansion bay then it's definitely a version 5 or 6.

There are definately 3 screws in the expansion bay... there are 2 gold ones like right in the expansion bay and there is a sort of square hole in the expansion bay where there is a silver screw. So, I am hoping this means for sure it is a V5.

If it is a V5, should i get a magic2 or apple chip?
