Sounds like bad media.

Try a Ritek disk.

Is it a DVDR or a DVDRW? If the later a lot of players are a bit fussy with rewritable media, Have you tried it on a DVDR (write once)

As geobon says .. some standalones and DVD-ROM drives can be choosy, when it comes to DVDRW's.
The Pioneer DVDRW's are very good disks, so I wouldn't think this is the problem.

Did you try playing the disk in your burner ?

It's just a compatibility issue then.
PS2's can be very fussy .... try a Ritek disk and it should work fine.
